Completed Fitchburg State College Application to which a $25 processing fee check made out to Fitchburg State College is attached (fee is non-refundable)

Catherine Leahy Brine, Inc. Cover Sheet

Official, unopened transcript from your undergraduate degree program (have all transcripts sent to C.F. Brine Box 1060, Brockton, MA 02303)

Documentation of a minimum of three years of teaching experience (Use the DOE Verification of Employment Form)

Documentation of a valid teacher certification/license at the Initial Level

Documentation of passing scores on the Communication and Literacy subtests of the Massachusetts tests for Educator Licensure (MTEL)

Professional resume, signed and dated

Three Letters of Recommendation on the official Fitchburg State College forms provided in the application; one must be from a supervisor; the other two from people who know you professionally. These names are also listed in item # 20 of the application packet

C.A.G.S. CANDIDATES ONLY: Official, unopened Masters Degree transcript to be mailed directly to C.F. Brine, Box 1060, Brockton, MA 02303

M.ED. CANDIDATES ONLY: You Must Take Either the GRE or the MAT before you start the program. Tests may be taken at any state college or at any Prometric testing Center See list of Prometric Centers Official scores of these tests must be sent directly to Fitchburg State college.

You must provide proof of having taken the test to Catherine Leahy Brine, Inc.before you start the program. Mail proof to C.F. Brine, P.O. Box 1060, Brockton, MA 02303. Proof includes one of the following: a. a copy of your results, b. your canceled check, c. a copy of your charge statement, d. a receipt from the testing center.

NOTE: This program does not have elective space for transfer credits. Up to four courses within this program may be taken prior to matriculation with permission of Catherine Leahy Brine Inc.
